A diagnosis of glaucoma that requires continuous medication.

With documented incapacitating episodes requiring at least one, but less than three treatment visits for an eye condition during the past 12 months.

With documented incapacitating episodes requiring at least three, but less than five treatment visits for an eye condition during the past 12 months.

With documented incapacitating episodes requiring at least five, but less than seven treatment visits for an eye condition during the past 12 months.

With documented incapacitating episodes requiring seven or more treatment visits for an eye condition during the past twelve months.

Localized scars, atrophy, or irregularities of the retina, unilateral or bilateral, that are centrally located and that result in an irregular, duplicated, enlarged, or diminished image.

Vision in one eye 20/40, vision in the other eye 20/40.

Malignant neoplasms of the eye, orbit, and ednexa (excluding skin) that require therapy that is comparable to those used for systemic malignancies (ex. systemic chemotherapy, X-ray therapy more extensive than to the area of the eye, or surgery more extensive than enucleation.
