Small, reducible, or without true hernia protrusion; or not operated, but remediable.

Postoperative recurrent, readily reducible and well supported by truss or belt.

Small, postoperative recurrent, or unoperated irremediable, not well supported by truss, or not readily reducible.

Large, postoperative, recurrent, not well supported under ordinary conditions and not readily reducible, when considered inoperable.

Wounds, postoperative, healed, no diability, belt not indicated.

Small, not well supported by belt under ordinary conditions, or healed venral hernia or post-operative wounds with weakening of abdominal wall and indication for a supporting belt.

Large, not well supported by belt under ordinary conditions.

Massive, persistent, severe diastasis of recti muscles or extensive diffuse destruction or weakening of muscular and fascial support of abdominal wall so as to be inoperable.

Documented history without daily symptoms or requirement for daily medications

Documented history of esophageal strictures that requires dialy medications to control dysphagia otherwise asymptomatic.

Documented history of recurrent esophageal strictures causing dysphagia which requires dilation no more than two times per year.

Documented history of recurrent or refractory esophageal strictures causing dysphagia which requires at least one of the following: dilation three or more times per year, dilation using steroids at least one time per year, or esophageal stent placement.

Documented history or recurrent or refractory esophageal strictures causing dysphagia with at least one of the following symptoms present: aspiration, undernutrition, or substantial weight loss and treatment with either surgical correction of esophageal strictures or percutanious esophago-gastrointestinal tube.
